A summons for jury service and a juror qualification notice are the initial documents that call you, as a prospective juror, to service. The qualification notice is sent to citizens to complete the online Juror Portal Questionnaire to determine if they are qualified to serve as a juror.

After logging in to the Juror Portal and completing the questionnaire you may upload a document on the Jury Home page letting the local jury office know what type of assistance you would need.

Below are electronic forms developed and approved for use in Missouri courts. Court staff are happy to help you if they can. However, courts staff are allowed to help you only in certain ways since they must be fair to everyone.

Individuals in some Missouri counties occasionally are subjected to phone calls threatening arrest and fines for allegedly missing jury duty. Please be advised these calls are scams seeking to obtain confidential information.
